What we do
Starting a new job? For an employment physical, we complete your employer's form and can add the TB test, drug test, or vaccines it asks for.
Nursing school and college physicals
A nursing school or college physical exam often comes with a list of titers — blood tests that check whether you're immune — a TB test, and vaccine records. We offer:
Titers
MMR (measles, mumps, rubella), varicella (chickenpox), and hepatitis B.
TB tests
PPD skin test or QuantiFERON blood test.
Vaccines
Tdap (tetanus, diphtheria, whooping cough), MMR, varicella, and hepatitis B.
A PPD skin test needs a short return visit 48 to 72 hours later to read it, at no extra charge.

Your physical exam is $150 flat, and we don't bill insurance for it. TB tests, titers, and vaccines cost extra — see the work physical price list. There's no fee for cash or debit.
The lab bills titers and other bloodwork separately: to your insurance if you have it — your plan decides what's covered — or to you if you don't.
Your visit
Bring your form, and read what happens at a pre-employment physical before you come.
Lab results arrive in Northwell's Labfly app, usually within 2 days — QuantiFERON usually within 5.
If you need to come back for a missing test, there's no second office fee.
Your doctor
Dr. Slava Fuzayloff, DO, examines every patient here himself; he doesn't hand you off to a physician assistant or nurse practitioner.
